DBS Application Request Form

As an organisation we are dedicated to safer recruitment (see safeguarding policy), therefore we ask new volunteers to under go a new and enhanced DBS check. The cost of this is to the volunteer at a very low price and the volunteer keeps the DBS arcus does not keep it.

Here is a list of documents that are required to apply for your DBS there are 3 groups. One from each group is needed. Please upload into upload field.

GROUP 1
Current Valid Passport
Current Biometric Residence Permit (UK Issued)
Current Valid Driving Licence Photocard- Full or Provisional (UK, Isle of Man
& Channel Islands Issued
Birth Certificate- Issued at Birth
Adoption Certificate (UK and Channel Islands Issued)
GROUP2
Current Valid Passport
Current Biometric Residence Permit (UK Issued)
Current Valid Driving Licence Photocard- Full or Provisional (UK, Isle of Man
& Channel Islands Issued
Birth Certificate- Issued at Birth
Adoption Certificate (UK and Channel Islands Issued)
Non-UK Current Driving Licence Photocard (full or provisional)
Current Driving Licence- Full or Provisional-Paper version (if issued before
1998)
Birth Certificate- issued after time of birth
Marriage/Civil Partnership Certificate
Immigration Document/Visa/Work Permit
HM Forces ID Card (UK Issued)
Firearms Licence
Mortgage Statement (UK Issued/ Issued in last 3 months)
Bank or Building Society Statement (Issued UK or Channel Islands/ Issued in
last 3 months)
Bank or Building Society Statement (Countries outside UK/ Issued in last 3
months & branch must be where applicant lives & works)
Bank or Building Society account opening confirmation letter (UK
issued/issued in last 3 months)
Credit card statement (UK issued) Financial Statement, e.g., Pension/Endowment UK issued (within the last 12
months)
P45 or P60 Statement (UK or Channel Islands issued)
Council Tax Statement (UK or Channel Islands issued)
Letter of Sponsorship from future employment provider
Utility Bill (issued in last 3 months)
Benefit Statement, e.g., Child Benefit, Pension
Central or Local Government, Government Agency or Local Council
Document giving entitlement (issued in UK within past 3 months)
EEA National ID Card (must be valid)
Cards carrying the PASS accreditation logo (UK, Isle of Man or Channel
Islands issued/must be valid) Letter from Headteacher or College Principle (UK issued/ 16–19-year-olds in
fulltime education/ only used in exceptional circumstances if other documents cannot be provided/must be valid)
Irish Passport card (must be valid/cannot be used with Irish Passport)
